We are committed to protecting the confidentiality and security of your personal information and personal health information in accordance with applicable Ontario privacy legislation, including the Personal Health Information Protection Act, 2004 (PHIPA), and the professional standards of the College of Massage Therapists of Ontario (CMTO).

When you book an appointment or receive care at NUOVA, we may collect information such as:

  • Your name and contact information

  • Date of birth or age

  • Health history and relevant medical information

  • Information about your current concerns and treatment goals

  • Information needed to provide safe and appropriate Massage Therapy

  • Appointment and treatment information

  • Payment and insurance information

  • Other information reasonably necessary for your care

We only collect information that is reasonably necessary to provide and manage your Massage Therapy care.

Your information may be used to:

  • Provide and document your Massage Therapy care

  • Understand your health history and treatment needs

  • Communicate with you about appointments and your care

  • Assist with insurance claims where applicable

How Your Information Is Collected

Information may be collected through your online booking, electronic intake forms, communication with NUOVA, and during your appointments.

NUOVA uses Jane and other appropriate electronic systems to assist with booking, intake, recordkeeping, and practice management. Reasonable safeguards are used to protect your information when these systems are used.

Please avoid sending detailed health information through an ordinary website contact form or regular email unless specifically requested.

Your Privacy & Consent

Your personal health information is kept confidential and will only be collected, used, or disclosed as permitted or required by law.

You have the right to ask questions about how your information is handled and, where permitted by law, to request access to or correction of your health records.

Consent to the collection, use, or disclosure of your personal health information is separate from your consent to receive Massage Therapy treatment. You may withdraw consent where permitted by law.

Sharing Your Information

NUOVA will not sell your personal health information.

Your information may be shared when necessary for your care, with your consent, or when permitted or required by law. This may include sharing information with other healthcare providers when appropriate and legally permitted.

NUOVA takes reasonable steps to protect your personal information and personal health information from unauthorized access, use, disclosure, loss, or theft.

Your information is only accessible to individuals who require it for authorized purposes.
